    1-on-1 Rehabilitation

    A hybrid approach combining in-person sessions with telehealth. Get personalized guidance, tailored exercises, and real-time feedback. This model is flexible, holds you accountable, and is focused on helping you regain movement, function, and confidence.

    You’ve been through the rehab cycle, you know the exercises…but something is still not connecting to your real life. This program helps you turn strength, balance, and progress into meaningful activity. We blend principles of neuroplasticity with lifestyle to make change.

What to Expect

    Evaluation

    60-90 minutes 1:1 evaluation on-site. Includes in-depth patient interview, evaluation of skills impacting daily tasks. Assessments include vision, motor performance, movement, strength, and coordination. The therapist will also perform in-depth analysis of daily activities and performance.

    Treatment Sessions

    45-60 minute sessions in-person or via Telehealth at your home, work, or in the community following a specialized treatment plan and interventions designed for your specific needs.
